Bookings closed Saturday 7^th October 2006 as the workshop is now fully subscribed.

This booking form constitutes a legally binding contract. In the unlikely event of cancellation, the organisers will make a full refund but disclaim any further liability.

Your invoice can be paid in the following ways:
	Cheque or banker's draft, made payable to the 'The University of Bath'. Please send cheques with remittance to the University of Bath and ensure the invoice number is written on the back of the cheque.
	Credit card (Visa or MasterCard only). This site does not offer secure payment facilities. To ensure security you can contact Natasha Bishop tel: +44 (0)1225 38 6256 or fax: + 44 (0)1225 386838 with your credit card details. When faxing, we need your name as it appears on the card, expiry date and card number. Your payment cannot be processed without your invoice number.

Cancellation Policy

All bookings should be received by 20^th October 2006. Cancellations up to 20^th October will incur a 50% cancellation fee. For cancellations received from 21^st October to 27^th October there will be a 75% cancellation fee. There will be no refunds for cancellations received after 27^th October. A substitute delegate is acceptable but please ensure that you notify Natasha Bishop of any changes prior to the workshop.
