Booking Form

This is the booking form for the Initiatives & Innovation: Managing Disruptive Technologies workshop which will be held at the University of Warwick on Friday 24^th February 2006.

The cost is £105 per person.

The delegate fee includes full attendance at the workshop, refreshments and lunch together with access to the venue's WiFi network.

the final scheduled date for bookings was Friday 17^th February 2006. The bookings actalluy closed on the morning of Tuesday 21^th February 2006.

Please use a separate form for each individual application. If a hard copy of this form is used, please return it to Natasha Bishop at the address below.

This booking form constitutes a legally binding contract. In the unlikely event of the conference being cancelled, the organisers will make a full refund but disclaim any further liability.

How your invoice can be paid

Your invoice can be paid in the following ways:

Cheque: cheque's are to be made payable to 'The University of Bath' send cheques with remittance to the University of Bath. Please ensure the invoice number is written on the back of the cheque.
Credit Card: to ensure security please Fax your details on (+44) 01225 386838 or call Natasha Bishop on (+44) 01225 386256/4488. Your payment cannot be processed without your invoice number. When faxing we need your name as it appears on the card, expiry date and card number. The University of Bath only accept Mastercard or VISA.
Bank Transfer: please note there is a £7 bank charge for this, which is extra to your booking fee. The University of Bath's bank details can be found on your invoice. Ensure your payment refers to your invoice.

Cancellation policy

Please note that cancellations up to 17 February 2006 will incur a 50% cancellation fee. There will be no refunds for cancellations received after 17 February 2006. A substitute delegate is acceptable but please ensure that you notify Natasha Bishop of any changes prior to the conference (details below).

It should also be noted that bookings should be made prior to the event - it is not possible to book a place on the day of the event.
